To submit an application:

  • Print out the form

  • Fill out all 4 pages

  • Be sure to include:

    • Your PRAXIS scores if you do not meet the endorsement requirements through your degree or 24 credit hours.  Please note, that for Elementary or SPED you MUST take and pass the Praxis Test Series #5901 before you submit your application.

    • Your resume​

    • Your cover letter

    • 3 Letters of Recommendation

    • Your transcripts

    • $30 application fee payable by cashier's check or money order.
